  • Medical

  • Birth Certificate (from Local Civil Registrar)
  • Verification of Birth Certificate (from jims)
  • Marriage Contract (Local Civil Registrar)
  • Verification of Marriage Contract (from jims)
  • if High School Graduate – (diploma with form 137)
  • if College Graduate – (diploma with transcript)
  • Voter’s ID or Certificate from COMELEC Intramuros
  • Other IDs (atleast 3)
    • #ex.(postal/BIR/Philhealth/Old Company or School ID)
  • Old Contract for ex-abroad
  • ARC (if ex-Taiwan)
